Why Naive RAG Fails in Production

Building a demo of Retrieval-Augmented Generation (RAG) takes less than an afternoon with modern Python libraries. You take a few PDFs, split them into 500-token chunks, compute embeddings, and search with cosine similarity.

However, once deployed to real users, naive RAG often fails in subtle ways: retrieving chunks that mention keywords out of context, losing table headers, or missing crucial historical qualifiers located in separate sections of a document.

The Power of Hybrid Search and Cohere Re-Ranking

To achieve enterprise-grade retrieval precision, standard practice is pairing dense semantic vector search with sparse keyword search (BM25). Dense embeddings excel at conceptual meaning, while BM25 guarantees that exact SKU numbers, proper nouns, and regulatory codes are never missed.

Passing the top 25 results from hybrid search through a cross-encoder re-ranking model (such as Cohere Rerank or BGE-Reranker) compresses the most relevant 3-5 passages to the top of the context window, dramatically reducing hallucination rates.

Automated Evaluation with the RAG Triad

You cannot improve what you cannot measure. Implement automated evaluation benchmarks using frameworks like Ragas or DeepEval before shipping to production. Continuously track Context Precision, Groundedness (Faithfulness), and Answer Relevance against a curated golden dataset on every prompt or embedding change.
